CHADIS is an online screening tool used before well-child visits.
Before a checkup, parents complete age-appropriate questionnaires and note concerns they would like to discuss. The provider can review responses before the appointment.
To complete CHADIS, parents and guardians should log in to the Patient Portal using their OWN credentials, not their child’s credentials. If you do not have portal credentials or have trouble signing in, call the practice where your child receives care. On the dashboard, find “FILL MILESTONES” at the bottom of the page and select CHADIS. See the step-by-step CHADIS instructions if you need help.
