Patient Information

CHADIS

Complete your child’s developmental, behavioral, and health questionnaires before the visit through the MAPP patient portal.

Before your visit

What is CHADIS?

CHADIS stands for the Child Health and Development Interactive System. It is an online screening tool used before well-child visits. It includes age-appropriate developmental, physical, behavioral, and mental health questionnaires.

Completing the questionnaires from home gives you time to answer carefully and allows your child’s provider to review the results before the appointment. CHADIS is used for screening at well-child visits, including age-appropriate screening for teen depression.

Three simple steps

How to complete your CHADIS questionnaires

1

Open the portal and Sign In

Use the MAPP Patient Portal link. The same portal serves both THH Pediatrics and Metropolitan Pediatrics.

Parents and guardians should log in using their own Patient Portal credentials, not their child’s credentials. If you do not have portal credentials or have trouble signing in, call the practice where your child receives care.

2

Scroll down to Fill Milestones

Once logged into the portal dashboard, find the FILL MILESTONES section near the bottom of the page. You may need to scroll down to the bottom depending on your device and screen size.

3

Select CHADIS

In the Fill Milestones section, click the CHADIS button to begin the questionnaires.

Please note that the requested questionnaires should be completed prior to your appointment.

Important

Use the patient portal above to complete pre-visit questionnaires. The standalone CHADIS website is not the correct place to complete them for your MAPP visit.
